Medical Plan

Heathnet of Oregon
  • Office Visits: $30 co-pay with in-plan providers
  • Annual Deductible: $3000
  • Health Reimburse Account: Reimburses up to 80% of any deductible expense above $500 to a plan maximum of $2000 for each participant.
  • RX coverage - Tier 1 - $15, Tier 2 - $30, Tier 3 - $50
  • Well Net Benefits - $20 co-pay for chiropractic, acupuncture, naturopathic and massage therapy services, maximum $500 per year.

Dental Plan

The Standard
  • Prevention – 100%
  • Basic Services – 80%
  • Major Services – 50% - not payable within the first 6 months of coverage

Vision Plan

VSP
  • $20 co-pay every 12 months for exams
  • Lenses – (some restrictions) every 12 months
  • Frame - $130 allowance every 12 months
